early mild cognitive impairment(mci) amnestic type on december 21, 2010
this is a precursor to alzheimers disease
because of this diagnosis i have opted to stop practicing medicine
this blog will be about my journey with this disease

alzheimers news-i think he had it

ALZHEIMERS NEWS-I THINK HE HAD IT

i know an author who wrote several books about him
says he didnt have alzheimers disease when he was president

well
i think
ronald reagan was in the early stages of the disease when
he was president
he had to have been

i read that his doctors back then in the 80s said he showed
no evidence of alzheimers
did he have memory tests like trump had earlier this year
the one i have had several times over the last 8 years
the moca test
no reagan didnt have that since it was developed in 1997
the other one used commonly is the mmse
it was developed in 1975
it could have been given to reagan but i doubt that it was

if you are early in the disease the mmse misses the diagnosis
the moca test usually doesnt

so my guess he was never ever really tested while in office
even by his private doctors

i was a primary care doctor having graduated in 1982
and finishing my residency in 1985
starting private practice in 1985
i will admit i never recall actually diagnosing alzheimers
disease in my career
it was something like cancer you didnt talk about with
patients
it sort of got ignored

my guess is that his symptoms were ignored
and not tested for

ive watched reagan on tv clips since i was diagnosed
and now knowing a lot more about the disease

you can hear it in his speech
you can see it in his face
the stare
if you have had a loved one with this disease
you recognize that stare
you know what im talking about

there are three stages in alzheimers
prealzheimers with normal memory tests and abnormal spinal fluid and a positive amyvid scan
mild cognitive impairment with slightly abnormal memory tests and abnormal spinal fluid and a positive anyvid scan
alzheimers with abnormal memory tests and abnormal spinal fluid and a positive amyvid scan

if trump will develop alzheimers as he ages he would be prealzheimers now
he would be in the white area
i have mild cognitive impairment
i am in the pink area
reagan at the end of his second term probably had early alzheimers disease
he would have been in the grey area




this is the alzheimers graph
the graph of the typical late onset alzheimers patient
it shows the development of symptoms and testing that we
now have available

this graph was developed from data from the large research
study that i was in called the adni 2 study

he died at 93
he quit the presidency at 77
plot those points on the graph
he was first president at 70 years old
plot that point on the graph

if this was his path which it probably comes close to
at 70 he probably had early mci or mild cognitive impairment
if you had done the moca test on him it might have been abnormal
not like the good score trump had

he probably noticed things himself
maybe his wife
probably not others unless they knew him real well
if he had had a full neuropsychological test it would
have been there
its the gold standard memory test
his spinal tap analysis for the bad amyloid and the tau
would have been abnormal
his amyvid pet scan the one for the bad beta anyloid
the hallmark of alzheimers disease would have been
positive
maybe early changes would even been seen on a mri scan

so i think when he was first inaugurated as president
these would have been positive

i have been exposed in the last eight years to a lot
of folks who have mci and real early alzheimers
they function quite well
they work they write they give lectures they drive
they travel they do all their financial stuff
most folks dont even know

when he was first inaugurated
my guess he would have qualified to be
in the aducanumab study that i am in now

wouldnt that have been interesting
if a sitting president was undergoing a novel treatment
for alzheimers disease

it probably would have been the best thing that
could have happened to the alzheimers movement

but none of this was available

my guess is he functioned quite well in his first term
my guess is that he started to have issues in the end
of his last term
issues he noted
his wife nancy noted
his close white house staff really noticed
and
they were able to protect him
he did a good job at the end
but
had his disease been a faster progresser
it might not have been a good ending on that second term

so
i believe
knowing all i know now
that he could have been diagnosed while in office
alzheimers just doesnt pop up one day
its a usually slow gradual process that can take
like in him
30 years to fully develop and cause death

lets dont be ashamed if he did have it while in office

sb country n news-towers for worms

SB COUNTRY N NEWS-TOWERS FOR WORMS
recently i started researching the internet on how to
do worm composting

at the school garden i had taken a storage bin
drilled holes in it
then
made a composter out of it
adding all the greens and browns you need to make a
composter work
we added some worms to the mix
however
the bin was so heavy it was hard to handle
so
i decided to convert it to a worm composter
thus
the worm composter internet search

i love youtube videos

as i was searching i hit this link
about worm towers using 5 gallon buckets
and
putting them in your garden

i was intrigued i must say
bascially
you dig a hole in the garden the depth of the bucket
take the bucket and drill holes in it on the sides
and the bottom
i did 3/8 in holes
then
in the hole i dug in the ground
i added shredded newspapers
thats worms candy they love it
plus
i added some compost
then
i dropped the bucket into the hole
then
in the bottom of the bucket
i added more compost and shredded newspapers
then
i wet this down good
worms like it moist like a sponge after you wring it out
then
i added shredded food scraps
like veggies fruit bread rice pasta egg shells preferably crushed
no meat no bones no sauces no oils
then
on the top of the pile
i keep moist shredded newspapers
covered with a thick moist newspaper or cardboard
then
i put a lid on the bucket
in my case
i added a square concrete paver over the bucket
to keep in moisture
to shut out light
to keep out varmints
then
all i do is add clippings from the garden
removed plants as long as they arent diseased
as i go along
then
when the worms get this all composted up
it will take several weeks
the worm compost that is left is used to fertilize the garden
plus
the worms go out into the garden to dig around
loosening up the soil
fertilizing it as they go
then return back into the worm tower
the buried 5 gallon bucket
for more food

so i added 4 of these spaced 4 ft apart down the middle
of my garden which is about 20 ft x 10 ft

so now
we recycle all our food scraps in the buckets
or
donate them to the schools worm composter

is this the ultimate food recycling idea or what

lets not forget them

LETS NOT FORGET THEM

ive never really been around a big disaster before
except in the 90s when the jarrell texas tornado hit
jarrell and killed folks and sucked away a whole subdivision
down to the concrete slabs and sucked the hide off cows
some of the missing folks were never found

now my brother and i were driving back from arkansas
as we left waco heading south on i35 to home
the sky just sort of got weird looking like
if you have been around storms and tornadoes
you know that almost weird feeling you get inside
looking at it
its almost a 6th sensing it

little did we know that the tornadoes were just ahead of us
they thank goodness outran us down the interstate

folks we knew were affected
one was found in her bathtub hanging in a tree
some turned out ok

i had a lot of patients who lived in jarrell texas
who were directly affected or their families were
its a scar on their memories and on jarrell
that still lingers today

the next disaster that i have been close to is the
santa barbara montecito fires and the following mudslides
folks these disasters dont care how much money you have
what color you are what your sexual orientation is
whether you are male or female old or young
they are a nondiscriminating disaster that destroys
and kills those things in its path

i didnt know any who were killed here
but ive met a lot that were affected
we even evacuated out of here ourselves twice
once for the fire and once for the flood
luckily we were just a few miles away from the disaster
mainly we were affected directly by the smoke
we couldnt hardly breathe
easily taking care of by leaving

to see the destruction so close by
like the jarrell texas tornado
will remain with us forever

as i was thinking about this this morning
all i could think of was
puerto rico

thinking how lucky we are to be on the mainland
and to be an american citizen
oh thats right they are american citizens

these folks unlike the jarrell folks or the montecito
folks could only run so far
since the ocean was their barrier to escape
they did have warning
but what could they do about it
the montecito folks had some warning but many didnt heed it
when it hit them the floods there was no time
to escape
the jarrell folks had no prewarning time to escape
many like the montecito folks were doomed

the puerto ricans were sort of doomed too
where could they go
some lucky ones flew to the manland
but just a few

then like with jarrell and with montecito  there was
total destruction

if you live here you have seen the amazing work that the
government organizations have done
those amazing fire fighters the police the cal trans folks
the corp of engineers all the local help
amazing was the response
to the devastating disaster
so it was with the jarrell disaster

it seems that puerto rico hasnt fared as well on the recovery efforts
i try to ignore the partisan opinions on this
i just know that the recovery for whatever reason doesnt seem
to be going as fast as what ive seen before
even with harvey in texas

i just know that weve seen the heruculan work done around here
i wish some of that effort could be sent to
puerto ricos way

lets dont forget them
it just seems we have
