welcome to the organic green doctor blog

i am a family physician who was diagnosed with
early mild cognitive impairment(mci) amnestic type on december 21, 2010
this is a precursor to alzheimers disease
because of this diagnosis i have opted to stop practicing medicine
this blog will be about my journey with this disease
please feel free to follow me along this path
i will continue blogging on organic gardening, green living,
solar power, rainwater collection, and healthy living
i will blog on these plus other things noted to be interesting

alzheimers news-opening the blood brain barrier

ALZHEIMERS NEWS-OPENING THE BLOOD BRAIN BARRIER

the drug i just finished taking in the biogen aducanumab embark study at ucla is aduhelm or aducanumab a monoclonal antibody or mab that is given monthly as an infusion
the aduhelm enters the brain and removes the amyloid plaque from the brain
so more than likely after all the around 35 infusions i had should have removed from my brain most of the amyloid plaque that can be measured on an amyloid pet scan

yes im sort of in a treatment limbo waiting for something new to come along that has potential to help slow down the disease

probably unless something else comes along i will get retreated with leqembi based on whether and when the amyloid plaque reaccumulates

so i wait
i moving on with my life but im keeping a close eye on research

one recent research item was a study done where they blew open the blood brain barrier using a low level ultrasound via mri to open up the blood brain barrier
then aduhelm was given as an infusion

the brain has a barrier called the blood brain barrier
think of it as a wall the body has around the brain to protect the brain
it keeps out infections and drugs and chemicals from entering the brain

ultrasound can be used to open up the blood brain barrier for a short period of time which allows the drugs like aduhelm to be given and have easier access into the brain
thus working better and using lower doses

there are a lot of good drugs that cant break through that barrier
this is a novel way to get them into the brain
eg some cancer drugs and mental health drugs and other neurological drugs

the researcher blew open the barrier on a specific side of the brain
the aduhelm readily entered and removed amyloid plaque
the other side of the brain was not ultrasounded so they could compare the effectiveness of this approach
that side had little or no change in the amyloid plaques

the open blood brain barrier only stayed open for a short period of time

if this technique would work using aduhelm they probably would be able to reduce remarkably the dose of medicine used

this was a preliminary study using only 3 patients
it got a lot of press and excitement in the alzheimers world

this morning i read where leqembi will be given this way
it will be interesting to see their results

so
maybe when i get my infusions of say leqembi in the far future they can blow open my blood brain barrier

so
i just wait for now
