this is a precursor to alzheimers disease
because of this diagnosis i have opted to stop practicing medicine
this blog will be about my journey with this disease

will this help curb the losses

WILL THIS CURB THE LOSSES

the life expectancy of white males is dropping
especially in the south
some of that is from obesity and diabetes
the sudden death rate
well its from opiates

opiates are vicodin or hydrocodone oxycontin fentanyl
obtained legally via prescriptions
and
illegally off the streets
also it includes heroin
which is cheaper than the prescription stuff

thus
white folks be dying from this stuff
in maine and new hampshire and well everywhere

is the solution getting rid of heroin and the street narcotics
this stuff has been around a long long time
its still here

now many folks got started legally on this stuff
thanks to the medical community
apparently for susceptible folks it can take 5-30 days
to get hooked

when i ruptured my disk in my back i was given 30 days
of vicodin to take
i took it too since it controlled my pain
but
i can tell you in the back of my brain was this little
opiate devil saying dont get addicted
as soon as i could i got off them

some folks
in those 30 days would have got hooked
its really how their brains got wired

apparently there are lots of nerve pathways in the brain
that get destroyed when some folks use opioids
stopping the opioids leads to an overwhelming desire for
the stuff

so far
theres nothing to use to stop this urge for more
now
methadone replaces the opioids by controlling the dose
naloxone added to pain meds helps also
but
when its stopped
back comes the urge for more
its the urge that doesnt go away

now enter cbd
whats cbd
in marijuana there are two chemicals
thc thats the one that gets you high
cbd thats the one that doesnt get you high
its the one that is used to prevent seizures in those kids
who cant control their seizures any other way

the thc is the component that gives marijuana the symptoms
we all read about

cbd does not

a variation of cbd is about to be approved by the fda for use
in hard to control seizures

now this is important

there will soon be a legal prescription pill for cbd that doctors
can write prescriptions for hard to control seizures

now enter off label use
thats where doctors use drugs for other reasons that what they
are approved for

i did it
all doctors do it
there are a lot of medicines you probably took and your kids took
that were used off label
eg albuterol proventil for asthma wasnt approved for use in folks
under 12 for years
most of my prescriptions for asthma for albuterol were in kids
thats off label use

remember this

last night dr gupta on his cnn report on the use of marijuana
specifically cbd in controlling pain
showed taking cbd decreased or eliminated the use of opioids in folks
who were addicted

remember their brains were changed maybe permanently
something has to be used to cover those receptors
it seems cbd does that
successfully in most cases

you should read this article or watch the cnn video on this
to make your own opinion on this
could this be the answer to our opioid crisis

i suspect most of you who are reading this has used marijuana
in you lifetime
i have
now i havent used cbd by itself yet
but i plan to
why
it appears that cbd may cause the removal of the bad amyloid
from the brain
thats sort of what the infusions i am getting at ucla does
the cbd also acts to decrease the inflammation in the brain also
a big component of the destruction in the brain in alzheimers
i have nothing to lose

i think if i was addicted to narcotics like heroin or other opioids
i would try the cbd program
like with me and my alzheimers disease

they have only one thing to lose if they dont try it
their
lives

the federal government makes it hard to do the research that needs to
be done to show if this really is the answer or not

this transcends your political beliefs
death from opiate overdose doesnt care what your politics are
it affects all of us

i wonder though if this new cbd pill will be used off label
probably

i think ill go buy stock in that cbd  company today

its their our fault

ITS THEIR OUR FAULT
this permeates all of the corners of the us
the big cities the rural areas the poor states the rich states
everywhere
we are all affected by it
this
opiate thing

i was watching the civil war by ken burns the other night
i also have been reading a book about a civil war doctor
they just lined up those shot up soldiers
took the saw those surgeons did
sawed off above the injured area
wiped off the saw
grabbed the next leg or arm
sawed it off
sometimes for several days without stopping

now those guys usually died from infections
some survived
many got hooked thats right
on heroin or morphine

ok so they sort of had an excuse
we now in the united states today
we dont really have an excuse

why do we have this opiate epidemic
its
big pharmas fault
congresses fault
pharmacies fault
federal and state governments fault
and
yes
go walk into the bathroom
look in the mirror

thats you
its your fault
you and i have the power to vote
either we dont care
or
we arent enraged enough

heck
this epidemic has hit the core of america
the middle class white folks
usually when this happens
things get fixed

here is sort of the story of how all this happened
yes i was part of it
since
i wrote prescriptions
and
i vote

heroin and such opioids have always been around
peaking at times
like in those solders after the civil war

the opiates were used
to treat pain
to treat sorrow
to treat then the addiction

a few years ago
the federal and state governments decided doctors werent aggressively
treating peoples pain
so
they began to reprimand doctors for not writing for enough pain meds
the push began
even i who didnt over prescribe this stuff
felt the pressure to well over prescribe
i loosened up my prescribing
heck
its no fun to get drugged in front of the state medical board
with your own lawyer
getting your name in the news
getting your name on the medical boards website
he got reprimanded for not treating peoples pain

now
that never happened to me
but it did to others

we all felt the pressure

now that i think back
i do wonder if some of this was not some conspiracy by big pharma
to get us to prescribe more of their drugs
you know they have powerful lobbyists
who spend millions influencing those folks that we vote into office
they provide these good looking drug reps to sell the stuff
giving away all kinds of free stuff
like meals trips etc

then big pharm really jumped in strongly
pushing their narcotics
on tv ads
sending those reps aggressively to visit with primary care doctors
who they lied to about how these powerful narcotics worked
like hydrocodone and oxycodone
all of this increased the supply the flow of these narcotics
out into the public
initially all legal

then
eg people having orthopedic surgery got all hooked
they needed more drugs
they started seeking more drugs
they stole from others
they faked pain at doctors office
some even sold theirs on the black market

the middle man
those companys that distribute drugs in the us
even
started leaking drugs into the black market

the flow of drugs became almost like a flood

then
congress those people you and i elect
decided to tighten up
on all of this
to stop the flow somewhat

first they started going after the doctors again
making it known that we were all over prescribing
just the opposite of a few years ago

yes
we all felt the pressure
we all cut back
probably too much

heck
i had oral surgery recently and they didnt want to give me pain meds
sorry
i almost demanded them
i am glad i did
since
there was 2 or 3 days of severe pain that thankfully resolved quickly
sure glad i had those few pills around to help

so now
we have created a large pool of addicted folks
we have started to under treat pain again

where do those folks turn to for relief
addiction centers
these are becoming underfunded and the new health law doenst help either
then
its the streets

the narcotic supply was dwindling and expensive
so
enter heroin
its cheaper
works fast
hooks you real good

now we are back to the civil war era
we really havent progressed much have we

we didnt solve it then
we arent solving it now

sorry
i wrote all this
but
i dont really have a solution to present to you
except
we can all vote cant we

if they the politicians think they will be voted out
they will start doing what it takes to solve this problem
but
probably not before then

this seems to be one problem that affects both parties equally
its a non partisan issue

is it a fix

IS IT A FIX
as a physician its always hard to figure out whether
someone is faking or not when they come in for
pain medicines
some are so obvious they are faking
others are real good at it
real good

when i first started practicing the trend was not to give
out much of that stuff
broken bones post operative pain acute pain from
say an acute ruptured disk etc
it was always a fine balance between helping people with
bad pain and making them an addict or contributing to
an addicts addiction

then the trend was to give more pain meds since state boards
would reprimand you or put you on probation if someone
complained to them and the board deemed it that you
didnt give them enough stuff for the pain

theres not a test you know
sometimes its quite obvious like a broken bone
other times it was not so obvious like back pain and headaches

i always wondered what those red flecks were in that vicodin

now i had a migraine headache once i know that must be what
one feels like
excruciating vise like pain that was unrelenting
mine was from a severe case of iritis where you can get
inflammation that causes the pain
luckily i had a hydrocodone (vicodin) left over from some oral surgery
that yes it took two to relieve the pain
i cant imagine having them frequently the pain not the pill

then i had the acute rupture of the disk in my back
felt like i was having 10 babies or once  or a bowel movement
the size of a boulder
i couldnt have made it without the hydrocodone
lots of it

i was able to wean off it and even now when it flares bad
i could see how someone could easily want to continue it
theres pain the more you have  the more you need
then
theres the tolerance the more you take the  more you need
for it to work

then the dea and boards started go after the docs that wrote
too many prescriptions for pain meds
specialties like neurologists and neurosurgeons and orthopedists
and pain doctors
are big writers of that stuff

write some for me please if that bone is sticking out my leg
many of them got caught in the big crack down on drug
prescribing
a lot of good docs got their reprimands when they were doing what
was right
dammed if you do and dammed if you dont

now the feds have moved the hydrocodone from a class 3 to a class 2
its now in there with morphine
at least its not ranked as bad as marijuana at class 1
means its a bad drug huh that weed

now you can only get a 90 day prescription with no refills
no phone calling it in
you have to see a doctor to get a refill
in some states no nurse practitioner or physician assistant can write it

what does this mean
less pills on the market for resale illegally
less addiction i guess
less pain control for those who really need it
more people moving to heroin for the addiction
its cheaper and probably easier to get
the black market will figure it

i still never understood why it only came with aspirin or acetaminophen
or ibuprofen or cough syrups
its the acetaminophen in there that has caused most of the problems with
liver failure liver damage

so now we will probably create this large number of folks hooked on
heroin
maybe less people will get hooked on drugs to start with
some people with pain arent going to get proper pain treatment
ironically a med commonly used for chronic pain is a class 1 drug
and is illegal in the us and in most states
thats marijuana
many nfl players use it for this reason

my views have evolved over time on this drug issue
we should let more people have them
there should be places like in europe where the addicts can get
what they need
marijuana should be legal
im sure if it were i would be taking it nightly to aid with sleep
and as alzheimers progresses use it for anxiety that can develop
we have filled up the prisons and jails with folks who really arent
criminals
someone is getting rich off all this
drug companies prison companies drug dealers
let put an end to their profits
