i am a family physician who was diagnosed with
early mild cognitive impairment(mci) amnestic type on december 21, 2010
this is a precursor to alzheimers disease
because of this diagnosis i have opted to stop practicing medicine
this blog will be about my journey with this disease

i will continue blogging on organic gardening, green living,
solar power, rainwater collection, and healthy living

mothers day, post abbie, ad classifications

MOTHERS DAY
on sunday i will pause and think about the two mothers
that have been important in my life












this is my mother
she would be 90 years old
she raised 8 children
the wife of a baptist minister
she was loved by all of her kids and all that knew her
she was a nurse and loved her profession

she was devastated by alzheimers disease in the last stages
of her life but
we remember her today during the best times

the other person
is my wife she










shes a good mother and a good wife and
a good sport for allowing me to pick on her
in this blog

shes a special person
will be thinking of her on mothers day
love you

POST ABBIE

it is a special story
pass it on to those who would benefit from reading it
when you think you cant do something in life just think of abbie
you can do it
she did and does

AD CLASSIFICATIONS
there are three new levels in the new diagnostic classification
for alzheimers disease

pre clinical azheimers disease-you and no one else notices anything

mild cognitive impairment due to alzheimers disease-you and others notice changes

dementia due to alzheimers disease-those changes are more obvious

the preclinical phase can last for several years before symptoms start

the mild cognitive impairment starts subtly then advances to more
obvious symptoms after the process has been smouldering in your brain
for all those years

dementia due to alzheimers disease occurs when those processes start
damaging the brain and start snowballing in their effects
its the downhill roll that occurs with alzheimers disease


preclinical

the red line the 1st line
beta amyloid starts to form and accumulates in the brain

if my younger brother had  tests in his mid to late forties on his spinal
fluid
they would have found decreased levels of beta amyloid
and on a beta amyloid pet scan they may have found some accumulation of
beta amyloid in his brain

my beta amyloid in the spinal fluid is reduced
i dont know my results of the research beta amyloid pet scan but plan
eventually to have the amyvid pet scan for beta amyloid

the orangey line the 2nd line
the synapses those nerve connections in the brain start to get damaged
if you measure glucose metabolism in the brain at this stage you may start
to see some loss of glucose uptake on a fdg glucose metabolism pet scan

at the beginning of this he would still be working without noticing anything
and
later as it progresses up the red and orangey line he will began to notice
some subtle things that others wont notice

this is where i enter the picture with the very early noticing of things
my fdg metalbolism test is normal

the green line the 3rd line
the damage is done to the nerve cell
things unravel in the cell and release a substance called tau
which can be measured in the spinal fluid

as the green line progresses upwards the symptoms he notices become
more obvious to him but probably not to others
the beta amyloid would be increasing in his brain and decreasing in the
spinal fluid
his tau protein levels would be increasing in the spinal fluid
he was in the middle of his second career as a policeman

i entered above and this is where i am now
my tau protein is increased and is in the borderline range

mci

the blue line the 4th line
the damage is occurring and on a mri there starts with subtle
changes in volume in the brain in the area of the hippocampus
that controls short term memory

at this stage the symptoms are becoming obvious to my brother and to
those around him
he actually sought medical help but was mostly ignored

i am not at this stage yet but am just where the purple line the 5th line
is beginning

the purple line the 5th line
this is where things start to take off
cognition begins to show up as worsening

the black line the 6th line
this is clinical functioning
this is how well your are functioning in the real world

as this line takes off and crosses the 2nd dotted line is where my
younger brother was officially diagnosed
short term memory was a problem
getting lost driving was a problem
at this point spinal fluid would show high tau protein and
low beta amyloid
fdg glucose metabolism pet scan would show progession of the decreased
uptake of glucose by the brain
a beta amyloid pet scan (like the amyvid) would show more involvement
of the brain with the beta amyloid plaques
a mri would show some brain shrinkage and changes in the hippocampus

he would have to quit working at this point
their life just changed significantly

after this point the lines all continue to increase and things just
get worse

where exactly am i
im at the first dotted line and if i follow my younger brothers path
i am 5-10 years behind him
hope its the later number

busted, habitat, thanks

BUSTED
no i didnt get arrested
my brackets for the march madness got all busted up on
the first day
dang duke and mizzu
you really screwed up my brackets

then that guy who plays for syracuse got removed off the team
really screwed me up
i had picked them for winning it all

is it ok to pick a new winner

my sweet 16 is a sour 16
i picked correctly 9 of the 16
thanks to ky, indiana, baylor, michigan state, louisville, syracuse
ohio state, north carolina, and kansas for helping my brackets out

ohio how did they get in there
oops they have to play north carolina

my final four picks were duke michigan state syracuse and kansas
maybe ill get one right

HABITAT
i am still working on the habitat house
the future home owner is excited
its really looks like a house now
at least from the outside
inside we still have to finish the floors
do the final painting inside
the cabinets are almost all in place

estimated move in is about 1 month
my wife she and i will put in her keyhole square foot garden soon
will post pictures of it when its done as well as post
a picture of the house

you should consider volunteering at your local habitat where
you live
the ones who seem to really benefit a lot are the high school
kids and college kids who work there

sure makes me feel good about the future of our country
when i work with these kids
i know some of these will be leaders wherever they end up
living

i actually took the week off from blogging
and posted my most popular blogs from the last two years

the blogs were hard for me to read as i realized how i was
blindsided by all what happened and how much it really affected
me and how much it still affects me when i read about it

although im sad all this has happened im not sorry it happened the
way it did as i probably would be drifting along in life not knowing
until something happened that would be tragic for someone else
and for myself

the future is being planned and my wife she and i will be as
ready as we can be
it will be harder on her as i probably wont know whats going on
just what things to be ready for her
help her out when the time comes

bluebonnets, snakessssssss

BLUEBONNETS
as preditcted the bluebonnets will be good this year
especially as we have more rain coming this spring
here is a picture of a area of bare ground that bluebonnets
took root in











here is the same area 3 months later











this use to be a 3 foot circle of bare ground that the bluebonnets
have taken over and covered the bare area
this is how they work in nature to stabillize the bare areas to
keep them from erosion

here is part of my front yard that is mostly all bluebonnets
with some clover mixed it











this is how nature does grasslands
a lot of this was bare because of the drought

should be a beautiful showing this year

this is our first bluebonnet here at the country n











we hope to have thousands more
this is the year of the bluebonnet
be ready to tour the hill country this spring
as it could be awhile before they are this good

interesting that this bluebonnet planted itself in my crushed
granite driveway-the worst soil on my property
now have to drive around it so it can continue to grow

SNAKESSSSSSSS
ok my best medical school friends wife
if you are reading my blog you need to stop now
i know how much you are scared of snakes and snake stories

do you still have that realistic rubber snake we gave you for christmas
that year that came in its own aquarium and was wrapped
so pretty

i have written several blogs about snakes
they all seem to involve my wife she
some of the snake events i dont dare let her know about
she would not live here anymore if she only knew

once she was in our hot tub on our deck











she was sitting in the tub looking out towards the river just
enjoying herself in the mid day while i was working in the
yard to her left
see that rain water tank next to the hot tub

(thats our movie star tank that was in a movie made
in austin
it was painted to look like a corroded industrial tank
we managed to buy it after the movie was made and left
it as it is)

as i was working and she was chilling out i looked up and there
on the top of that rain water tank was a big ole chicken or rat
snake just coiled up looking down at my wife she in that tub

i kept my moouth shut and went on working in the yard
she never knew until i did my blog on this uneventful event

it would have been fun to watch though if they snake would
have fallen into that hot tub

talkin about streakin
that would have been some streakin to watch and of course to
listen to
or
she might have just had a heart attack there in the hot tub

recently in the blog world i made contact with a new blogger
she has only written two blogs
she is a photographer who is deathly afraid of snakes
like my medical school best friends wife (are you still reading this)
and like my wife she

her best friend has pet snakes
and wanted to have her photograph the snakes
so she did

she must be a brave lady

happy valentines day, bagels, keyhole garden



HAPPY VALENTINES DAY

hope every one has a happy guess thats what you say valentines day
today especially my wife she
she is always a good sport about me writing about her on this blog
sometimes i do pay a price for doing it

BAGELS

every morning i eat a bagel with a fresh organic free range egg that has
been microwaved
i even have a special microwavable dish just for microwaving eggs
i like the yellow part soft almost runny
it seems to have more flavor that way but
those holes are a problem in the bagels
they let the yellow runnnnnny part come through

so why do they have those holes in bagels anyway
so i googled it as they always have the answers
well some i cant post here but

the holes were originally removed because the middle part of the
bagel did not get cooked well so it was removed
and later the hole was used to place several bagels on a stick so they
can easily be presented for sale
so google says

guess ill just have to overcook that egg or plug the hole in the bagel
with something

KEYHOLE GARDEN




here is the photo of my keyhole garden i made out of an old
watering trough
i was computer challenged to get this picture off my phone so
my rain water tank nephew who really didnt fall into my rain water
tank helped me transfer this to my computer then got it posted

when i did it google didnt like it and wouldnt keep it posted
now maybe itll stay on my blog

in the center is a 1 foot circle of chicken wire that will be 3 ft high
its buried down to the bottom of the trough (about 2 ft)
inside this compost bin i placed shredded newspaper, my compost,
chicken poop, the natural gardeners farm style compost then a layer
of leaves and repeated this layering until its full
i will keep the center part watered well and all these goodies will
diffuse out into my garden

on the far side barely visible is a wedge like a piece of pie cut
from a pie that has rock stones and crushed granite that i can
step on to easily access the compost tower and the plants in the
middle of this garden

the metal bucket is a 10 gallon bucket that is attached to a drip line
that encircles the bed of the garden with about 1 ft distance between
the circles
this i fill with water and let gravity drip the water on the plants

i am thinking about adding the compost tower to the center part
of any 4 ft square foot garden i do for novice gardeners so they
will have a ready made compost pile thats simple to use and
easy to add more compost materials

christmas, giving or gifting

CHRISTMAS
nice christmas
spent it with my two brothers and their wifes
my historian brother he and my rainwater brother he

we potlucked it
actually had a nice vegetarian christmas meal
ok i did sneak in some christmas ham
made me a large pot of greens from the garden-turnip greens, kale,
asian cabbage, cabbage, spinach, swiss chard, onions, garlic
all organic
my wife she added her corn bread
all went well with the other food

my wife she and i had a private christmas earlier in the morining
where we opened our gifts together in front of our fire in our
wood burning stove
very picturesque

my proudest gift was a rogue hoe
now i have two hoes
this rougue hoe will last forever
sort of like your wife
it will easily be used in this rocky hard soil around here
cant wait to dig things up with it

overall a great christmas
hope to repeat it next year

GIVING OR GIFTING
have thought a lot about christmas this year
we do approach it all wrong
just watch tv at all the ads for stuff we dont really need
it should be about giving not gifting

my relative gave me a gift card this year
it was to www.kiva.org
the gift was for $25 and i went online and registered to kiva

kiva is a organization that gives microloans to people all over the
world usually to start or improve their businesses
they then pay the money back
there are very few defaults

the last one i did was to a guy in florida who needed $5000
to start his own lawnmower business
so donors of all over the world donate $25 a piece and kiva
loaned him the money
he has already paid 10% of it back in a few weeks

when these loans are paid back i will then re loan the money back
to some one else
my relative has tripled the amount of money he has loaned over the years
as he just continues to roll the money over into new loans

those dam banks would never ever loan these people this money
like the guy in florida
can just see shank of america loaning a new immigrant $5000 to
start a new business

give next christmas instead of gifting
so many of us and our kids already have too much as it is
give to someone who needs it more
or like with kiva give to those who are trying to improve themselves

my wife she

MY WIFE SHE
today is my wife she and my anniversary
its number 37

we met while students at a junior college in southern
california
she worked in the same hospital as a medical assistant
where i worked as a medical assistant in the emergency room

we also had classes together
i was her lab instructor for her chemistry class
after we starting dating she got an a :)

we enrolled at different universities our junior year
so just before the year started she transferred to the
one i was attending
we then went up to las vegas and was married there on
the weekend then returned to start school on monday

weve never looked back since
well maybe once or twice

we both graduated together
she was 8 months pregnant when she finished
we then moved across the country so i could eventually
go to medical school

she worked in the maternal and child health division of the state
health department the 7 years i was in medical school and
residency

after i started practicing medicine she worked as a mom
and did a lot of volunteer work in school
this is where she developed her desire to teach

12 years ago she got her teaching certificate and has never
looked back
she loves what she does and it shows
you know what i mean you can tell when people love the
job they are doing
just ask her former students, fellow teachers, parents, and the
administrators

my wife she and i are both lucky that we are and were able to
do as a careenr what we love to do
many never get that chance

so today on our anniversary i would like to thank my wife for
being with me the last 37 years and especially for being there
this last year with me on my new(our) journey
our second phase of our life
