HOPE FOR THE FUTURE

these kids and their generation and the generation
just above them
are the hope for the future
for everything but especially for
a cure or preventative treatment for
alzheimers disease

i can see it like way over there in the horizon
like when you travel in the dessert in the early evening
or early dawn
way off in the distant you see those glittering lights
and those areas of green

hope is out there but its way out there

the cure and treatment will probably come too late
for my generation
but
it will be there for these little guys and their soul mates

a new study i read about this morning was done on women
who took multivitamins with minerals long term
they had a 35% decrease in their risk of dying from
heart disease

now i know from all my reading on alzheimers disease
that generally whats good for the heart is good for the brain

there right now are so many exciting potentially game changing
studies with different new drugs and treatments


the A4 study
folks will be given the new alzheimers vaccine  who
have positive scans that show they will probably get
alzheimers disease but have no symptoms
the vaccine will remove beta amyloid from the brain
thats the bad stuff that is thought to be the culprit in
alzheimers disease
if it works they wont get the disease or will slow down its
progression


the apoe4 studies
folks who have the double apoe4 gene like i have
which puts them at high risk for developing alzheimers disease
will be given the alzheimers vaccine

other folks will be  be given a bace inhibitor which stops
the brain from producing the bad stuff beta amyloid


colombian studies
folks who have the dominant gene for alzheimers disease
and all yes all of them will get it in their 30s to late 40s
and die from it
they will get the vaccine also

so using the vaccine and the bace inhibitor will be thoroughly
tested and we will know in 3-5 years if this approach will
work
if it does we will see this form of treatment get fast tracked
to folks who are early like me or who are at high risk
like
eg my siblings who arent affected yet
they can be screened with scans and spinal fluid analysis
if they are positive
then they could be started on a lifetime of treatment

so maybe this is the hope for the future we are looking for

of course if it doesnt work we are knocked back to ground zero
but
there are lots of other studies being done


the noble t-817ma study just got fast tracked some because of some
promising new studies showing it may help
basically this chemical promotes the new growth of new nerve cells
and
inhibits brain cell death


the sniff study just started one that i might get involved with
basically insulin is sprayed up the nose
goes up the smeller nerve thats the olfactory nerve to the
part of the brain where alzheimers disease starts
it increases glucose to the brain cells some damaged and some not
preliminary studies shows that folks who use this
have an improvement in memory

i expect this will work some and will become part of a
treatment regimen for folks

so these studies above are ongoing now
in humans
and will be available in 5-10 years if they help

some hope for the future

there are lots and lots of studies in animals usually rats or mice
that seem promising
like the stanford study studies on multiple older meds like
citalopram bexarotene etc


then
there is the reversal of cognitive study from ucla that was published
last year that use 36 points
half were healthy things like blood pressure cholesterol diabetes control
exercise diet sleep mental activity meditation for me thats gardening
and making sure your numbers are ok like thyroid b12 homocysteine
vitamin d hormones
then the rest was supplements based on human and animal studies
that showed promising results

he had 9 of 10 people with memory loss get better
the one that didnt had more advanced alzheimers disease
most had memory loss documented on memory tests
some had abnormal scans
when they went off the regimen they got worse again
so
theres something in what is in that 36 point study

maybe some it will be related to the new study referenced above in
women with lower heart disease rates who took multivitamins with minerals

so
in summary
there is hope for the future
for
we always have to have hope dont we

what if

WHAT IF
for those of us who follow the alzheimers research
there are two things that we know about and are anxiously awaiting
the results

a4 study
colombian study

in both these studies
people in the a4 study who have
positive beta amyloid in the brain on scans called
amyvid pet scans and abnormal levels of beta amyloid and
tau levels in the spinal fluid
and in the case of the colombian study
family members who carry the gene for alzheimers that
means you will get the disease will also have these
two tests done

no one in either group have any symptoms yet

both studies will have regular amyvid pet scans and spinal
fluid analysis for the beta amyloid and tau
both imply if positive that the patient will get or will have
alzheimers disease

however

the study subjects will also be given one of the new alzheimers vaccines
which will induce or produce antibodies against
the beta amyloid and remove it from the brain

over time we will know if giving this vaccine early on
will prevent the development of alzheimers disease
which it will if the beta amyloid theory is correct

how does this affect me
the ogd

this last week i had my regular yearly visit with my neurologist
for follow up for
my early mild cognitive impairment amnestic type
that may turn out to be from alzheimers disease
diagnosed almost 3 years ago

i have been stable over this period of time except for some
mild problems with short term memory that only i notice

i presently take aricept (donepezil) 10 mg for the possibility
of having alzheimers dsease
its used when the diagnosis is first made or in my case
strongly suspected

i take care of my health
especially my blood pressure and cholesterol
2 of the 3 most important to control to prevent or slow down
alzheimers diesease
the third is diabetes which i dont have
more about this later

what i am looking for in research studies now

im in the alzheimers disease neuroimaging initiative now
its not a treatment study but its the most important one ever done
on alzheimers disease
the information from this study has provided the basis for the
a4 and colombian studies

i want to get treatment now while im so early no one notices anything
these two studies would be what im looking for

the colombian study is restricted to family members in colombia
that carry that awful gene which means they will get it
i know that i dont have that gene since ive been tested

the a4 does not
if i qualify i will get the beta amyloid scan amyvid and yes have
to get that dang spinal tap again and again
but i might receive one of those new alzheimers vaccine
that if it works would slow down the process at least while
i was on the study
unless of course i would get put in the control group
which i dont want to be in

if i dont qualify for that one
i will get in a nasal insulin study for 6 months
the nasal insulin increase the glucose into the nerve cells
in the brain where alzheimers starts as the insulin travels up the
olfactory nerve the smeller nerve i call it
which may slow down but wont prevent the disease
this may be my fall back study

then in 2014 or 2015 there is a study that will start in arizona

where those who carry the apoe4/apoe4 double gene like i have
which increases your chance of getting alzheimers disease significantly
will be given either a new research drug or one of the new
alzheimers vaccines
and
will be followed over a period of time monitoring the
beta amyloid scans and levels and the tau levels to see if they decrease
and to see if it prevents the development of the disease

the budget cuts and sequester had shut this study down
but the nih scrapped up funds to get it going

i am on the contact list for that study when it starts
i will do it if i qualify
but
please on please dont put me in the control group

will it be there in time

WILL IT BE THERE IN TIME
thats the question im asking myself
as ive read a lot about alzheimers disease and how it progresses
real gradually it seems over 20-30 years
and probably over a lifetime
it has multi causes most likely
changing some of those causes may help
you know like blood pressure weight cholesterol diabetes exercise diet
alcohol intake smoking sleep habits etc

as scientists are getting better and better at diagnosing alzheimers
disease we are and will  be seeing more and more  people diagnosed
with this eventually before any symptoms show up

now thats the dilemma
you can diagnosis them
but you cannt really treat them except by altering those things mentioned
above
or
use the 5 medicines approved to treat alzheimers disease
none of which actually treats the disease itself but only treats the
symptoms
eg
im on aricept (donepezil) and have been it for 2 1/2 years in hopes if
my final diagnosis turns out to be alzheimers disease that it may have
slowed down the progression of the diseases symptoms

of the 5 drugs only aricept (donepezil) exelon and namenda are
really used

what is needed now is drugs or treatments to treat the disease before
symptoms ever start and probably as much as 20-30 years before it
starts to show up

possible treatments attack the beta amyloid accumulation or the tau protein
unfolding and destruction in some way
these are thought to be the two hallmarks of alzheimers disease

ive written about the bexarotene studies that are starting up
hoping that the baxarotene will inhibit beta amyloid production
like it did in animal studies
(bexarotene is an old anticancer drug used for certain skin cancers)

in another study one of the old leukemia chemotherapy agents will be
given at a low dose to slow this production of beta amyloid
it appears to work in animal models

several companies have developed new drugs also that work like the
bexarotene and are being tested now in new studies that like bexarotene
will decrease the beta amyloid being produced in the brain

excuse me it seems that you could go back and study people who
were treated with these medicines for their leukemia and cancer and
see if it helped those who were going to get alzheimers
i bet there are blood samples and spinal fluid samples stored somewhere
that could be looked at

the big colombian study using one of the alzheimers vaccine that binds up
the beta amyloid and removes it or prevents it from being deposited in
the brain
hopefully this works because if not this approach using injectible
antitbodies against beta amyloid will not be effective
in this sudy it is given to people in their thirties who are going to get the
disease in a few years
so we will know in less than 5 years if this approach will work

right now across the us another alzheimers vaccine is being given to
prodromal alzheimers patients thats people some like me who have
positive mild neuropsych tests positive beta amyloid and tau proteins
in the spinal fluids and positive pet scans for beta amyloid but who
are really asymptomatic

i would qualify for this study but one of the criteria is that you can not
be on aricept (donepezil) and be on the study
thats a choice i would have to make
would be to stop this knowing
that if this vaccine didnt work stopping the aricept (donepezil) might
make my symptoms deteriorate if what i have is alzheimers disease

as of right now im not willing to do that

so im watching the drug studies closely for a potential treatment that
is in the works

one promising drug is being evaluated by taurx.com where the drug affects
the unfolding of the tau protein
earlier studies on a protype of this drug showed some promise

for me personally
i dont qualify because they arent testing it yet on people like me but are
only giving it to people who are in the early stages of alzhiemers
disease eg like the tennessee basketball coach pat summitt

ill be watching this one real close and if they do start a study in
people like me with minimal symptoms and positive tests
then i plan to try to enter the study
just dont give me that damn placebo

another study that i read about involves an enzyme thats called
secretase
it was discovered in a group of people in iceland who do not get
alzheimers disease because of a gene inherited that affects this
secretase enzyme so it doesnt break down amyloid into the
beta amyloid piece that is thought to possibly be the cause of
azlheimers as this beta amyloid piece gets deposited throughout the brain

a new study is being looked at using this to treat people hoping they
too will not get this disease
these new inhibitors of this secretase enzyme lower the beta amyloid
in the brain and
if it works will slow down or prevent the disease
this may be the most promising of all the new treatments
ill be the first in line for that one

yes this is hope for the future
these new drug treatments that are coming
but i realize my window of opportunity is closing if what i have is
alzheimers disease because by the time an official diagnosis is made
its like being told you have stage 4 cancer
the odds are against you

so
lets hope then that one of these new drugs will be effective
soon
