this is a precursor to alzheimers disease
because of this diagnosis i have opted to stop practicing medicine

alzheimers news-thanks bill

ALZHEIMERS NEWS-THANKS BILL

one of the goals of alzheimers researchers besides the main one of finding a  treatment or cure of this disease is to be able to identify folks with alzheimers early in the disease or even before symptoms start

if you can get to folks before symptoms start or before damage has been done
then you have a better chance of slowing or stopping the disease

once the damage is done
its done
its not repaired

thats why you see so many drugs fail
they are started too late
they cant fix whats already been broken
its like a vase dropped from a roof onto a sidewalk
its smashed to thousands of pieces that cant be put back together

so it is with an alzheimers brain

so if i really have alzheimers disease then its being going on for at least 15-20 years
it just hasnt taken over enough brain cells yet to make me have noticeable symptoms
that damage cant ever be repaired even if they find a treatment or cure

so thats why early detection then treatment even if there is no symptoms is so important

ive read for the last 8 years since i was diagnosed
that an early blood test to tell you if you have alzheimers is almost here
they seem to say that each year
its not here yet

enter bill gates
one of the richest men in the world
who has given lots of his money away for good causes
basically he got his money off us all who bought his products and now he is giving it to more needy folks

he recently donated well over 100 million just for alzheimers research
now
he has donated 30 million just to find an affordable test like a blood test for early detection of this awful disease
here is his statement

“Imagine a world where diagnosing Alzheimer’s disease is as simple as getting your blood tested during your annual physical.”

well i can imagine it
this money was put into a venture fund
he calls it diagnostic accelerator

we the federal government should be doing this by spending our money for a disease that is programmed to totally break our healthcare system in the near future
it will cost us billions of dollars

thanks bill
for doing what our federal government wont do for us

he is the definition of a philanthropist


on another note
biogen will be presenting data form the aducanumab study that i am in and one similar to it on sunday at a large alzheimers conference
expect to see news reports on their findings

expected approval for use by the fda if its approved will be 2020 or 2021
please hurry since i am scheduled to come off the aducanumab study in spring 2020
i dont want to lose anything ive gained

this aducanumab will not be a cure for alzheimers
it will be a treatment that will slow it down

alzheimers news-2 more to go

ALZHEIMERS NEWS-2 MORE TO GO

yesterday at the ucla infusion center i received #18 of my infusions of biogens aducanumab the drug that is given iv and is an antibody against the beta amyloid protein thought to cause alzheimers

it removes the amyloid plaques from the brain
it also removes any free floating beta amyloid in the brain
it also activates the garbage cells called microglia that clean up debris that accumulates
so
it works several ways  which is different from the other alzheimers amyloid antibodies being studied

as i explained to the infusion nurse yesterday i chose this study to be in
i identified it when information about the drug was first published
i watched it carefully

i tried to get in one study called the a4 study using another experimental antibody called solanezumab
which was given in this study to those who dont have symptoms but have positive pet scans for beta amyloid the hallmark of alzheimers disease
these folks are called prealzheimers
so i didnt qualify

when i initially tried to get into the aducanumab study i was turned away
that didnt stop me from trying though
i even foregoed my last amyloid pet scan in the adni 2 study i was in for 5 years starting in 2010
since i knew that doing that scan if i tried to get accepted into the aducanumab study would disqualify me since there was a rule that if you had a recent scan you couldnt qualify

once they obtained all my records and prescreening info though they accepted me in the study especially since their amyloid pet scan was positive

so im either prealzheimers or i have mild cognitive impairment thats due to alzheimers disease

hopefully whatever i have this stuff will probably help in someway to slow the disease down
we should have results in a couple of years that will tell us if it does

so yesterday as i left
i realized that i have two more of these left to do
before i advance to the final stage in three months
thats when all of us who want to continue will all get the real drug for another 18+ months
none of us will be on the placebo

now
i dont know if i got the placebo or the real drug
i had a 67% chance of getting the real drug
i was willing to take those odds knowing that after these 18 months
that my chances of getting the real drug are 100%

so yesterday as they ran the saline flush in my vein after the infusion was donei thought
man i wonder how im going to feel in 2 months when they flush that last one

ill be a happy guy for sure

i have now had 18 infusions into my vein on my left arm
the last 3 times they have had difficulty starting my iv in that arm
so they switched to the right arm this time

i was saving that arm for the round two that starts in october
but
i decided yesterday i dont really care where they put it as long as im getting the real stuff
heck i told them they could put it in my neck vein if they wanted

the biogen stock
biogen makes this drug
has shot up a few times recently
i think the financial world knows something about this drug
plus they have a another sister drug thats further behind in the research world in a phase 2 study that is also showing promise

so maybe
just maybe
all the effort my wife she and i are putting in may be worth it

alzheimers news-predicting your chances

now i know my chances of getting alzheimers disease
its as near 100% as one can get

what about you

my memory has been tested multiple times over the last 8 years
enough that when i do the tests
i know how well i am doing with my memory

over 1 year ago i had a several session evaluation of my memory
to get qualified for the biogen aducanumab study that i am in now

this last wednesday as i wrote in my wednesday blog i did my
1 year followup exam
then
in 6 months ill have it repeated as we end the first part of the
aducanumab study

so
when they started asking me my questions and having me
draw stuff and identify things i knew when i was finished
where i stood with my memory
i dont need no report from the study to know where im at
i know
i have not worsened in the last year
at best i may have improved
but
its not worsening that makes me feel better about things

but
what if you
those of you who have no symptoms
what if there was a way to know if you were going to
get mild cognitive impairment the stage right before
full blown alzheimers
some researchers thinks its the first stage of the disease
thats where i am at right now

but
what if you could be tested

here are the stages of alzheimers disease

prealzheimers   normal memory
mild cognitive impairment mci   mild memory loss
alzheimers disease  memory loss

researchers have found testing that is over 90% accurate
by testing small changes in multiple genes that when they
are all added up can predict whether someone years
before the disease shows up as memory loss
will develop mild cognitive impariment

its called polygenic risk assessment

its available in research now

lets just say that if thats the testing folks would have done in the future
those of you who are really worried
like i was in my 50s
i wanted to know
then
if this becomes available then you could find out
if you will get mild cognitive impairment ie early memory loss

then
what if a treatment for this was available
then
you could be treated years before the disease was
programmed to start
then
you might not get alzheimers

its amazing how it seems that little pieces of the puzzle are
being discovered every day
hopefully
the whole picture can be all fitted together one day
so we can prevent folks from getting this disease

in the meantime until all this becomes available
if you are worried you will get it
then
eat right
quit smoking
exercise
get adequate sleep
be active mentally and socially
control your health like diabetes obesity cholesterol blood pressure
relax
enjoy life
so
you will be ready when the treatment gets here

alzheimers news-dontcha remember

well thats sort of what im being asked to do today
see what i remember

i know i wrote after the alzheimers panel i was on a
couple of weeks ago
dont use the word remember is what those affected said

well today i have to test my memory skills
at the one year follow up for
biogens aducanumab research study at ucla
aducanumab is an antibody against amyloid which attaches
itself to the amyloid and removes it from the brain
its given as a monthly infusion

one of the indications that it is working is
an improvement or no change in ones memory

to get in the study
you have to undergo a lot of testing
labs scans neurological exams and memory testing

so one year ago there was a lot of memory testing
memory testing is the part of the research study that
i dont like
i call it sort of torture testing

if you have any memory issues
its bothersome real bothersome to do the memory testing

so one year ago the original testing is the baseline
memory testing that future testing
gets compared to

i had memory testing about 6 months ago
it was not as comprehensive as the beginning testing

from looking at the schedule today i probably will
have about 2 hours or so of memory testing

if this stuff is working
if i am getting the real drug and not the placebo
my memory will not have worsened
it may not  have improved
it may have just stayed the same
which is acceptable to me

i read today that alzheimers disease may be like hiv
you will be able to take medications that will stabilize
the disease so that it doesnt takeover the brain
thats ok with me
thats acceptible

my guess
my memory hasnt changed
its hard to tell if it got better since im so early into the
disease as it wasnt bad to begin with

ok i admit trump had a better moca memory test score than i did
on my first one

heres hoping i do remember things

also
i have to have a repeat liver enzyme test
they were slightly increased two weeks agao
i think because of the excessive acetaminophen and ibuprofen
and naprosyn along with some hydrocodone i took while
fighting the flu and the dental abscess
i do expect those to have settled back down to normal
heck
when i had my molar tooth removed last week and
had a bone graft done
i opted to skip the pain meds completely
they wanted me to alternate the ibuprofen and acetaminophen
i smiled
i didnt do that
i did take a couple of pain pills the first night but none since

there is nothing i want to interfere with me being in the study
nothing
it has the potential to be life changing for my wife she and i

so
im one year in
i have at least 2 plus years to go
in 6 months i will definitely be getting the real drug
aducanumab

i think my remembering is ok

alzheimers news-i agree

when i read what she said
i turned to my wife she as she drank her morning tea
as she sat
in what i call our spin around chairs in the morning sun
looking out over the courtyard of our condo

i said
did you read that article
did you see what she said
i agree
i told her

please be the drug

thats the title of this time magazine article this week
on
inside one couples experimental treatment to battle
alzheimers disease

lets say before i go further
these studies are available all across the usa to those
who have early alzheimers and mild cognitive impariment
and those who dont have memory issues yet

these as of right now may be the drugs that will slow down
the disease

when reading this article and like this weekend on the
alzheimers panel i was on with other patients
i am definitely no where near the stages that they are at
this couple is on infusion #15
i am on infusion #13

the stuff called aducanumab is an antibody thats given monthly
it was discovered in elderly folks who didnt get alzheimers disease
the lab produced it in large quantities to be used in this study

it works differently than the others you may read about
its more aggressive at removing the amyloid plaques in the brain
than the others
in the initial study the amyloid was removed and the study subjects
memory improved

after the 18 month infusion
for me that will be in september 2018
i will definitely start receiving the real drug for at least 2 more years

i like peter hope that i am getting the real drug
but
like him if not
im ok with it
at least im early in the disease
earlier than he is
my our participation hopefully will make a difference in the
future for others

all this was not available ten years or so ago when my
younger brother was diagnosed
or
15 years ago when my mom was diagnosed
there was no potential treatments out there being researched
that had any promise

now there are quite a few
that work differently to slow things down

there are 2700 of us in the study across the world
i think we may be the lucky ones to get treated so that the
alzheimers disease gets slowed down
we be the first to have this happen
ever

2/3 of us are getting the real aducanumab
1/3 of us are getting the placebo ie saline

i feel like im getting the real drug based on side effects i have
i sure hope so
as
what a difference these 3-4 years of treatment could make in
our lives
my wife she and i

sometime in about 2020 when all this is published i might actually
find out whether i got the placebo or not the first 18 months

some thoughts as i read this article and watched the video
if you are worried or have a loved one with it
you should read it and watch the video

ok so i cried twice
once watching the video
i could see my wife she and i sitting on that bench looking
out at the water wondering what lies in the future for us
and
then again when i read the article

he mentions driving
he cant drive by himself as he gets lost
ive made the decision to not be a statistic
getting injured in a wreck or injuring someone else
or
being one of those elderly alerts that shows up on the interstate
nope
if i have issues
im done
take my keys

he carries the genetic gene the apoe 4 gene for late onset alzhiemers
25% of you carry that gene
it increases your risks about 5 x for getting alzheimers disesase

i carry the apoe 4/apoe 4 gene the double version or homozygous
3% of you carry that gene
it increases your risks about 10-20 x for getting alzheimers disease

to get into this study you have to have a positive amyvid pet scan
for beta amyloid in the brain the hallmark of alzheimers disease
im in the study so you know what that means for my scan

the national institute of health nih has been given a doubling of their
alzheimers research funding
bill gates from microsoft has given millions for research now since
his dad has alzheimers disease
mark zuckerberg from facebook has also agreed to fund millions
for alzheimers research
both are emphasizing the mid level alzheimers researchers since thats
where all the drugs and probably the treatment will come from
so
as they hung this infusion #13 this last week at ucla
my lucky number
under mary easton alzheimers centers guidance
i looked at the nurse
i said
i want the real deal

she looked down at me
smiled real big
and
said
it is

alzheimers news-alzheimers resolutions

ALZHEIMERS NEWS-ALZHEIMERS RESOLUTIONS

many of us do the new years resolutions thing
i did it this year
but
didnt write them down
i just did them in my head
you know
the usual stuff we all write
many of the things we never do

however
there are resolutions that i do every year
they are almost absolute ones
they are
my alzheimers resolutions

my number one ranked one
the one that may possibly have the most impact
is
continuing in the aducanumab study at ucla
it ends in september 2018 for me
theyll look at the studys results
if they look promising
all 1000+ of us in the usa will be given the chance
to continue for another 2+ years on the real stuff
the real drug aducanumab

if it works as advertised
it will slow down the disease
how much we dont know
i do feel like i am getting the real drug
so
i plan on making sure getting this drug has priority
in my life
my wife she agrees

then there are the others

i will continue to talk to anyone who asks me about alzheimers disease
in fact i will tell you more than you want to know

i will eat a healthy diet like the mind diet
i will drink 1-2 glasses of red wine at night
i will drink 3 cups of coffee in the morning
i hope to improve sleep i addressed this this week now that weed is legal here
at least for now unless jeff gets a wild hair
i will stay active physically mentally and socially
i could improve on the physical thing some since i am not
working every day on our property in texas
i will plan to increase the activity

i am happy
i am not sad
i try to have my moments of zen each day
sitting here in the condo or standing in the garden or on the beach  etc
i will continue to stay on my 36 point regimen of healthy things
and supplements from dr bredesens reversal of cognitive decline article
i will update and solidify my will my power of attorneys my healthcare
 directive my legal records etc

so
if i can do all those things
i think i am doing the best i can with my resolutions
to slow this stuff down

yes
i make resolutions every year
but
mine may be a little different than yours

alzheimers news-do you match

recently i was contacted by the alzheimers association for
an interview
they wanted to know more about the trial match program

i was contacted by a writer from the los angeles area who wanted
to interview me about the trial match program and other ways
folks could get involved with the research studies

what was to be a 30 minute interview turned into an hour interview
with a later call back for more information
we discussed the trial match program and the nih clinical trial program
then
he asked a lot of questions about my story and how i was diagnosed
and how i am doing now

since it had been over 7 years since i had entered my information
in the alzheimers association trial match program
i signed my wife she up to review how it was done
the link is www.alz.org you click on research
there is the trial match tab
which i clicked on and entered her information

now she doesnt have any risk really for alzheimers disease
however
she matched with close to 40 studies in the california area

yes
they need  healthy volunteers
many are used as the controls for some of the studies
they need caregivers or potential caregivers for studies
they need folks for healthy living studies

as i discussed with the writer this week
the two big limits on alzheimers research
that is slowing down our discovery of a treatment and or cure
are
money
volunteers

who would have thought that not having enough volunteers
would keep us from finding a treatment or cure

yea
who would have thought

no matter where you live
there is a study that you will qualify for that will be nearby

eg
some of the drug companies that have a drug similar to the one
i am on (aducanumab)
have spread their study sites almost uniformly across the us
so that most of you are near a treatment site

if you are over 50 and share my dna you may qualify for these studies
where people will be treated before the disease shows up
many of you are 10-20 years away from the symptoms showing up
then
sorry to say if you wait until then its really probably too late

without volunteers the research will not be done fast enough

the study i am in hasnt filled up yet
even after a year
and
this is the most promising study that has the potential to be the
first drug that will actually slow the disease down
whats the limitation
getting folks to volunteer

this is the point i hope he makes when he writes about the
interview
is that we need folks to volunteer via trial match
or clinicaltrials.gov to help us in our fight to find a cure

yes
it takes a commitment to do this
but
it wont get done if you and others dont get involved

so
even if you are in doubt
got to trial match and sign yourself up
even if all you do is see what studies you qualify for

alzheimers news-the alzheimers week that was

ALZHEIMERS WEEK-THE ALZHEIMERS WEEK THAT WAS

it started with our santa barbara walk to end alzheimers last saturday
i was doing ok helping set up the walk
since i am on the committee for the walk this year
now
i was doing ok
until i had to get up and talk for a few minutes
since they asked me to carry the blue flower
the blue flower represents those of us who have alzheimers

now
ive got up and talked before a lot of these walks
like this
in fact i pretty much say the same things each time

this time
as i stood on the stage
looking out over the crowd of close to 500 folks
my granddaughter looked up at me
i could see her pretty little face with this look on it
granddaddy she said out loud
what are you doing up there

well
that was when the dam broke
on my tears
which flowed all the way through my two minute talk
it flowed from me to the audience to the other speakers
to the two emcees

i managed to get through it
barely

i guess folks will remember me talking after all that

then
this week was our ucla trip
to get my infusion #9 of the experimental drug aducanumab
that hopefully will slow the disease down some

for these trips
its only 90 miles
but its into the beast of the 405 in la
a drive that can take 2 hours or 3 1/2 hours
sorry
but this day we got the long version

we leave while its dark
after about an hour we start hitting the beast of the 405

the last 15 miles takes longer than the first 85 miles

so we eventually get there
i get settled into my bed i have to lay on
they start my iv
then
draw all this blood work
then
i see the neurologist
then
i wait for the pharmacy to mix it all up
the pharmacy knows what im getting
no one else does

i always tell them
please dont give me that placebo one
on you know you got the real one the nurse says
she says that to everyone i bet
then
they hook me up to the iv
it takes an hour to run in
i feel nothing but a salty taste in my mouth
after an hour they flush it with saline
then
im out of there 3 hours after it starts

my wife she says as she sees me
you know
your face is always real flushed when you finish
those infusions
yes
i said
any side effect i can get is a good one

i have somewhere around 50 +- more of these to get
hopefully a lot more

then
yesterday the biogen folks presented the results of
their phase 1a study that had been extended for two years
on around 150 folks

this phase 1a study was the one that looked promising
the fda allowed it to get fast tracked to phase 3 studies
so if it helps it can reach the market quicker
the phase 3 study is the one i am in now

this phase 1 study when they extended it these two years
is pretty much like the study i am in

so those folks some of them have been taking the aducanumab
now for 3 years
so these results give us a glimpse into whether this stuff will work

basically in layman terms
my terms that is

the 10 mgm dose worked the best but also had more side effects
so in my study they only are using the 3 mgm and 6 mgm dose
so if i am not getting placebo then i am on one of these doses
when my phase 3 study gets extended in 15 months
i will definitely be on one of these doses

here is what they found in layman terms
after 3 years the aducanumab removed enough of the beta amyloid
from the brain so that the amyvid pet scan for the amyloid
 are now mostly negative
or close to negative on the lower doses

now
the big determinant of whether this works is
does the memory get better or does its decline slow down

now this is my interpretation of listening to the webcast of the presentation
and reading articles on the results

none of the doses stopped the decline in the memory based on
two memory tests one being the mmse
but
the high dose the 10 mgm
slowed the decline of the memory significantly compared to the
decline seen in the folks who got the placebo
the 3mg and 6 mgm also slowed the decline in memory
but not as much as the 10 mgm
lets just say it did it half as much

so
in alzheimers years
3 years can be a long long time
and any slowing of the mental decline over the 3 years
i know any family member would take that

compare that to aricept (donepezil) the alzheimers drug most folks
get put on at first
there is no comparison to the aducanumab

so
my laymans impression
it removes the bad stuff the amyloid from the brain
by doing so the aducanumab slows down the memory loss
enough that if it does this it will be used in the future to
treat someone with early alzheimers

also
my impression is even though the amyloid is removed
there is destruction and some disease process thats going on
that has already started snowballing and may not be stopped
that may be why the memory decline isnt stopped completely
but
man i would take this stuff knowing that
it could give me extra time with my family
time that i would remember also while its happening

just think
if your loved one could have gotten something like this
they might have
made a wedding
maade an anniversary
seen a graduation
seen a birth

yes
i think it would have been worth it

alzheimers news-#8 done and more

ALZHEIMERS NEWS-#8 DONE AND MORE

we got this now
is what i told my wife she as we were zooming down the 101
turning onto that notorious 405
we are about to enter the mouth of the beast
i told her
i was talking about the traffic not the study i am in

its a 1.5 to 3 hour drive one way to ucla from where we live
how long it takes is based on the traffic and the time of day
i must say for our 800 am appointment we just zoomed right there

when we were going to ut southwestern in dallas for my
aducanumab study to get my infusions
it was usually a 3-3.5 hour drive
so
we dont really complain about the la traffic
well we do but we shouldnt since we get to la quicker
than we did to dallas

so this aducanumab im getting as an infusion is made by biogen
it removes the bad beta amyloid that starts the
alzheimers cascade of events that leads to the brain cell
destruction that causes the symptoms and signs
that you see in your loved ones

the theory is removing the beta amyloid may make a
difference

it seems from what i read this morning that wall street
likes biogens chances of producing a drug that will
alter alzheimers disease
its interesting that sometimes wall street knows things
we dont know

then this morning i read about dr tanzi from harvard talk
about his research on being able to grow a minibrain
in a petri dish that did what they predicted

first beta amyloid was produced
then brain cells died
plaques fomed
then tau proteins unraveled
then inflammation occurred
then the minibrain started dying

this is what the theory of amyloid causing alzheimers says
it just got proven in a petri dish

now
if this aducanumab stuff works like it did in the early studies
the phase 1 study
it removed all of the beta amyoid after a year in all of those in the
early study who had received the aducanumab
the placebo folks had no change maybe just it got worse

so if this stuff aducanumab really works
i and others will get better

as least wall street the stock market thinks it might work

the first results of our study should be out in 2019 or 2020
by then i will have gotten if i got the real stuff which i think
i am getting
at least 3 years of the drug before the results come out

in alzheimers years 3 years is a long long time

another article i read which sort of sucks is that the fda may
make them use it in folks who already have symptoms
who may not be helped as much by this new drug
it
to really be effective like a lot of the new drugs have to be given
to folks who do not have symptoms yet or who like me have minimal
symptoms
so that this amyloid stuff doesnt accumulate in the brain
even i may be too late too far along to get the maximum benefect
from this drug

still
i want to be the first person to proudly hold up this flower
at the alzheimers walks in the future

the fall this time of year
i tend to do a lot of reflecting
i guess because thats when all the alzheimers walks occur
when you see a lot in the paper on tv and social media
about alzheimers disease

i start thinking a lot about our family

a nurse who was struck with this dreadful disease
later in life
it was sad to see her deteriorate so slowly

my father charles
he died of heart disease but probably had early alzheimers disease

my older brother charles was diagnosed 3 years ago
he seems to be stable for now

a retired deputy sheriff
he was struck with the disease much too young
in his early 50s
he was gone in his early 60s
i think about him a lot

when i was diagnosed in december 2010
i really thought in september 2017 i would be where he was with his
disease at that time

i feel so lucky that i am able to sit here today
writing this blog
doing all the things i did when i was diagnosed
7 years ago
i really didnt think i would be able to sit here this morning
and
write
and
drive
and
do all our finances
and
know everyone
and
travel
and
see my granddaughter start to grow up

i feel lucky
maybe its from
all the healthy stuff i do
all the protocol stuff i do
the medicine aricept (donepezil) slowing this down
maybe
just maybe

if i am  lucky the aducanumab i am getting each month
an antibody infusion against the bad beta amyloid in the brain
the stuff that may be the cause of alzheimers
might slow things down some

i know from preliminary studies that if i an not getting the
placebo but the real aducanumab that  most of that stuff the amyloid
will be removed by next year
the question is
will the memory stabilize or get better for those of us getting
this experimental drug
if it does work to improve memory
then its part of the answer
then
ive gained 5 years
which if you have had a love one with this disease like we have
thats a lifetime of time

i think about all the folks i have met over the last 7 years
patients husbands wives sons daughters grand kids cousins friends
all like our family deeply affected by this disease

i know many of you who are reading this blog
your family has been affected also

i will be thinking about my family and yours and theirs
as i and my family and my friends and former patients
do our walks this fall to raise awarenes
and
to raise funds to educate and finance research and encourage
our government to do more

tomorrow here in texas
there will be a time
when i will stop
go inward
think of my family and others
yes i always tear up
then
i will move forward
to do what you have to do to make things better
